In choosing this animal, I agree to the terms, conditions and policies established by the Friends of the Mobile Animal Shelter (FOMAS). I agree to: *

1. Provide food, water, shelter and humane treatment for this animal at all times.

2. Provide annual veterinary exams, health care (as needed and/or recommended by my veterinarian), flea control and heartworm preventative and vaccinations required by law.

3. Prevent this animal from running at large - dogs must be on a leash or kept in an enclosed area; cats must be kept indoors.

4. Not allow this animal to be used for experimentation.

5. Not use this animal strictly as a guard dog.

6. Accept full responsibility for this animal's temperament and personality.

That I am 18 years old or older.

That if I live in an apartment I have paid any additional security fees and any additional rent.

I understand that if I can no longer care for this pet in the manner set forth in this agreement that I must return the animal to the Friends of the Mobile Animal Shelter (FOMAS).

I understand that FOMAS reserves the right to refuse adoption to prospective owners who fail to provide accurate information on the adoption application

I understand that FOMAS reserves the right to reclaim said pet if conditions of this agreement are not met.

I understand that cats and kitten must be taken home in a carrier or sturdy cardboard box.

I understand that I will have the proper facilities to maintain and contain this pet before placement.

That all pets currently in the household have current veterinary care.

Cats will NOT be declawed. Dogs will not be left tethered unattended.

I understand the pet I am adopting has been examined by a Veterinarian but all conditions may not have been detected at the time of the examination. *
